AHA Photoshoot - RESCHEDULED Sunday, March 1, 2026
STEP 1: Fill out the model release form below.
STEP 2: Select “Mandatory Fee” package, and add to cart.
- All athletes participating must purchase the $20 booking
- If you have two or more athletes participating, please make separate transactions and fill out separate forms.
STEP 3: Select the package(s) you would like to purchase, and add to cart.
- All athletes participating in the photoshoot must purchase a minimum of Package #1.
STEP 4: Once all package(s) are added to your cart, view cart and complete payment.
